Hearing that you’ve had an abnormal Pap smear can be unsettling. For many women, the first thought is, “Do I have cancer?”

Fortunately, the answer is usually no.

An abnormal Pap smear simply means that some of the cells collected from your cervix look different than expected. In many cases, these changes are minor, temporary, and caused by conditions that resolve on their own. The purpose of cervical cancer screening is to identify these abnormal cells early, long before they have a chance to develop into cancer.

Understanding what an abnormal Pap smear means, what happens next, and how your OB/GYN monitors cervical health can help ease anxiety and ensure you receive the appropriate follow-up care.

What Is a Pap Smear?

A Pap smear (also called a Pap test) is a routine screening that checks for abnormal cells on the cervix, the lower portion of the uterus that opens into the vagina.

During the exam, your healthcare provider gently collects a small sample of cervical cells using a soft brush or spatula. The sample is then sent to a laboratory where specialists examine the cells for changes that could indicate precancerous conditions or, less commonly, cervical cancer.

Pap smears are one of the most effective cancer screening tools available. Since their widespread adoption, cervical cancer rates and deaths have declined dramatically because abnormal cells can often be identified and treated before cancer develops. The American College of Obstetricians and Gynecologists (ACOG) and the U.S. Preventive Services Task Force recommend routine cervical cancer screening based on age and individual risk factors.

What Does an Abnormal Pap Smear Mean?

An abnormal Pap smear does not automatically mean you have cervical cancer.

Instead, it means that some cervical cells appeared different from normal under the microscope. These changes may range from very mild to more significant, and many never become cancerous.

Abnormal results can occur for several reasons, including:

  • Human papillomavirus (HPV) infection 
  • Mild inflammation 
  • Recent infection 
  • Hormonal changes 
  • Natural cell changes that often resolve without treatment 

According to the Centers for Disease Control and Prevention (CDC), HPV is the most common cause of abnormal cervical cell changes. In fact, HPV is so common that nearly everyone who is sexually active will be exposed to it at some point during their lives. Fortunately, most HPV infections clear naturally within one to two years.

Understanding Common Pap Smear Results

Pap smear reports use medical terminology that can sound intimidating, but many findings are relatively common.

Some of the most frequently reported results include:

ASCUS (Atypical Squamous Cells of Undetermined Significance)

This is the most common abnormal result. It means some cells appear slightly unusual, but it’s unclear whether HPV or another cause is responsible.

Your provider may recommend HPV testing or repeating the Pap smear in the future.

LSIL (Low-Grade Squamous Intraepithelial Lesion)

LSIL usually reflects mild cellular changes caused by HPV. Many LSIL findings resolve without treatment, particularly in younger women.

HSIL (High-Grade Squamous Intraepithelial Lesion)

HSIL indicates more significant cellular changes that have a higher likelihood of becoming cancer if left untreated. Additional testing is usually recommended promptly.

Although these terms may sound concerning, they help your OB/GYN determine the most appropriate next steps, not predict cancer.

Note: There are other, less common Pap test results that may occur.  If you have any questions about your results or what they mean, consult with your doctor or midwife.

What Happens After an Abnormal Pap Smear?

Follow-up depends on several factors, including:

  • Your age 
  • Your Pap smear result 
  • Whether HPV testing is positive 
  • Your previous screening history 
  • Your overall health 

In many cases, no immediate treatment is needed. Instead, your provider may recommend:

Repeat Pap Testing

Some mild abnormalities simply require another Pap smear in one year to determine whether the cells return to normal on their own.

HPV Testing

HPV testing helps determine whether high-risk strains of the virus are present. This information helps guide decisions about additional evaluation.

Colposcopy

A colposcopy is a common office procedure that allows your OB/GYN to examine the cervix using a specialized magnifying instrument.

If an area appears abnormal, a biopsy may be taken for further evaluation.

Although the word “biopsy” sounds intimidating, cervical biopsies are typically quick procedures that provide valuable information for diagnosis and treatment planning.

When Is Treatment Needed?

Many abnormal Pap smears never require treatment because the abnormal cells disappear naturally.

However, if high-grade precancerous changes are identified, your provider may recommend removing the affected tissue before cancer develops.

ommon treatments include:

  • LEEP (Loop Electrosurgical Excision Procedure) 
  • Cervical conization (cone biopsy) 
  • Cryotherapy in certain situations 

These procedures are designed to remove abnormal cells while preserving as much healthy cervical tissue as possible. Treatment decisions are individualized based on the severity of the abnormality, future pregnancy plans, and overall health.

Can You Prevent Abnormal Pap Smears?

While not every abnormal result can be prevented, there are several ways to reduce your risk.

These include:

  • Staying current with routine Pap smears and HPV screening 
  • Receiving the HPV vaccine if eligible 
  • Avoiding tobacco use 
  • Practicing safer sex 
  • Attending all recommended follow-up appointments 

The HPV vaccine protects against the strains of HPV responsible for most cervical cancers and is recommended for adolescents and many adults through age 26, with some adults ages 27–45 also benefiting after discussion with their healthcare provider.

Don’t Skip Your Follow-Up Care

One of the most important things to remember is that an abnormal Pap smear is only one step in the screening process.

The greatest risk isn’t receiving an abnormal result. It’s delaying recommended follow-up.

Regular monitoring allows providers to identify changes early, often before symptoms ever develop. When precancerous cells are treated promptly, cervical cancer is highly preventable.
